It's a somewhat depressing end to the tale, with an entire race simply fading away from the planet.

The Men, Hobbits, and Dwarves of Lord of the Rings could live there, and they have, but when their time comes to see Mandos, their souls are moved into the mysterious afterlife for which Tolkien gives no description.
The Hobbits stay for a for a time in Minas Tirith before returning to the Shire. Aragorn later spends much of his time in Arnor, from where he occasionally came to visit the Hobbits on the edge of the Shire, since by his own laws he (a man) could not enter it.
