The gore was done practically, which is always appreciated from horror movies as CGI blood has nothing on practical blood in terms of feeling authentic. Unfortunately, some nice-looking blood doesn't completely forgive the poor CGI found in other aspects of Malignant.
Realism: If a scene requires a high level of interaction with actors or a realistic feel, practical effects may be the better choice. Complexity: If the desired effect is too complex, dangerous, or impossible to achieve practically, CGI may be the only viable option.
Because Gabriel can't use Madison's eyes (again, she's in that trance) and his are located on the back of her head, the walking-talking-murdering teratoma twists his body to see and strike simultaneously.
