Presumably, the life of Thor mirrored that of his Earth-616 counterpart before the outbreak of the zombie plague. Like all Asgardians, he became a zombie once Odin had been infected with the zombie virus.
Unable to gain access to the fortress, presumably, Thor gets held up by the risen Deadite Legion, and at some point returns to New York, where he ends up with a handful of other zombified heroes. Later on, in the fight over eating the Silver Surfer, Thor is reduced to a charred skeleton, thus ending his undead life.

On Earth-2149, Mjolnir is wielded by a zombified Thor. In this reality, Thor is no longer worthy to lift Mjolnir (presumably for being a zombie) and thus has adopted a replacement hammer, made of unknown materials.

Sandman ended up killing all the zombies except Sentry. Uatu then sent the Sentry through time and he arrived back on Earth-2149, closing the time loop and containing the virus on Earth-Z and 2149 since it cannot be destroyed.

In God of War, Kratos throws the weapon into the Lake of Nine and awakens the World Serpent, who rises out of the water. Jormungandr returns the Axe to Kratos by spitting it out and, in the process, covering it with his venom.
Thor's Prosthetic Eye is a cybernetic prosthetic eye given to Thor by Rocket Raccoon as a replacement for Thor's right eye, which had been gouged out by his sister Hela.
It's unlikely to return. The loss of his eye is symbolic of becoming king of the Asgardians. In Norse mythology, the price of obtaining wisdom or power is the loss of a body part. Thor, having become wise enough to save his people, at the loss of the realm, paid the price by losing his eye.

Found by the Acolytes, he would rule over New Wakanda and eventually be turned before the Zombies' return. Sent to Earth-Z, he was eventually killed in battle trying to stop the infection.
