The twist that the real Brahms was alive and there was nothing supernatural is what made it work, but The Boy II bizarrely goes out of its way to undermine all that. It posits the real Brahms was nothing but a puppet for the doll, and the character seemingly died following the events of the original.
It appears that The Boy is another horror movie about an evil doll. However, in a twist, Brahms is still alive and hiding in the house's walls, pretending that the doll is possessed to remain hidden and safe. The twist makes the story scarier and makes Brahms a frightening villain.
However, the spirit of the doll inhabits Jude as he dons the real Brahms' mask. The real Brahms isn't shown in the film, heavily implying that he died from his injuries in the first film, as Joesph, the mansion's groundskeeper, revealed that he rebuilt the Brahms doll.
This releases the evil spirit, but the family still manages to defeat it. It's not until the concluding moments that the happy ending is removed and Jude puts on Brahms' porcelain mask. It appears that the spirit has successfully possessed him after all.
Liza goes to look for Jude but finds Joseph entering with a shotgun. He tells Liza that he put Brahms back together because he called to him, and he knew that Jude would be an easy target for Brahms to get ahold of.

Is the doll alive in The Boy 2?
Essentially, you discover at the end that the doll Brahms was never 'alive' to begin with – nothing supernatural was occurring. Instead, it turns out that a 30-year-old man has been living in the walls moving the doll around to make it seem like it's haunted.
In August 2022, Bell stated that he has plans for a third installment. The filmmaker stated that his intentions are to round out the character of Brahms, with the completion of a trilogy.
Brahms then began wearing a porcelain doll-like mask so as to hide his mutilated face. His parents, who by then had grown exhausted, and likely aware they could not care for him forever, hired countless nannies throughout his young adulthood in an attempt to find someone to care for him.

As it turns out, Brahms is actually a grown man living in the walls of his parents' British estate. The Heelshires had been hiding him away for two decades, since he murdered a young girl, and they brought Greta over from America as a parting gift of sorts.

However, the film ended with a huge whopper by revealing that there was nothing malevolent about Brahms the doll, as the real Brahms was still alive. It was a refreshing twist to the movie, but Brahms: The Boy 2 decided that the doll actually was evil after all.

Her dress and jewelry vanish while she is in the shower. She finds photographs of young Brahms and in each of them he looks sullen and never smiles.
