Samara, when alive, was a very powerful psychic child with a terrible sadistic streak. She could implant her nightmarish mental images into photograph film.
Eventually, Sadako visits Ikuma in the Izu sanatorium, only to be raped by a doctor named Jotaro Nagao, who is unknowingly infected with smallpox. During the assault, he discovers that Sadako has testicular feminization syndrome, and has the genitalia of both sexes.
Eventually, the sea demon impregnated Shizuko with a baby girl named Sadako. Shizuko intended to abandon the baby girl at a nearby cave but Sadako was still there, even when the waves tried to wash her away. Reluctantly, Shizuko raised Sadako as her own child.
A ghost, Samara was once a child gifted with the psychic ability known as nensha. As she was unable to control this ability, Samara would mentally affect those around her, including her adoptive mother Anna.
She experienced the bombing at the age of two, but grew up strong and healthy. Then, in the 6th grade, she suddenly developed leukemia. She hoped to recover, but her wish was not granted. After an eight-month battle with the disease, Sadako died at the tender age of 12.

What caused Sadako dead at age 12?
That is why it came as such a surprise when at the age of twelve, Sadako began to show symptoms of leukemia, and had to be admitted into the hospital. While in the hospital, Sadako remained optimistic and resilient. Even though Sadako was sick, she continued to bring happiness and cheer to her family and friends.

During her time in the hospital, her condition progressively worsened. Around mid-October 1955, her left leg became swollen and turned purple. After her family urged her to eat something, Sasaki requested tea on rice and remarked, "it's tasty". She then thanked her family, those being her last words.
In the Hiroshima Peace Memorial Park, there is a Statute of the A-Bomb Children. The statue is modeled on the young girl Sadako Sasaki (1943 – 1955). When she was two years old, Sadako was exposed to the radiation of the atomic bomb. She developed leukemia 10 years later and died at the age of 12.
Anna, out of sheer torment and maybe also out of concern for society, kills Samara by throwing her down the well. (Its interesting to note that Richard may have also been there helping since its unlikely Anna had the strength to move the stone lid over the well by herself).
One of Samara's powers is being able to project strange, disturbing imagery. It's believed that at some point during her haunting, she was able to imprint these images and her own rage onto a VHS tape kept in Cabin 12 right above the well.
Not yet even 20 years old, Sadako is murdered by a doctor at the sanitarium where she is kept following her mother's suicide. This doctor, entranced by Sadako's constantly noted physical beauty, first rapes her. Then, upon discovering she is intersex, throws her into a nearby well to her death.
back home, Aidan, who has psychic tendencies, tells Rachel that Samara will “never stop.” Indeed, Noah dies the next day, his seventh day. Rachel was only spared because she had made a copy of the tape, helping Samara circulate her evil. She forces Aidan to make a copy so he, too, may be spared.
In the story, Sadako's mother, Shizuko, is portrayed as a psychic with the abilities of clairvoyance and thoughtography, while her father, Iguma Hirahachiro, is portrayed as a scholar who was fired from a university after scandals about his research into Shizuko's abilities.
The video tape's curse was created by Samara likely after her death in the well. Having a miserable and lonely life, Samara was nearly drowned at birth by her mother Evelyn, nearly drove her adoptive parents mad, and was eventually pushed down a well to her death seven days later.
The only way to prevent Sadako/Samara from killing you in seven days after watching the her cursed video tape is to make a copy and show it to someone else (Sadako and Samara) or watch it backwards (Samara).
