Tracking the renegade clone to Level 1315, Fox and a contingent of clone shock troopers confronted Fives, demanding that he stand down. In his panic and desperation, the ARC trooper raised a blaster pistol at the approaching guards, but Fox opened fire, killing Fives with a shot through his chest.
He didn't know Fives, so all he had to go on were reports and what Palpatine told him. From his perspective, Fives was a dangerous traitor. Palpatine told Fox to kill Fives, it's unlikely he didn't after that little clip of Palpatine and Fox before Fox goes to that warehouse.
As Fives pleaded with his old friends, shocktroopers led by Commander Fox burst in and ordered him to surrender. Fives panicked and grabbed one of Rex's pistols, but was shot and mortally wounded. Dying, he told Rex the conspiracy was bigger than anyone could have imagined.
His words convinced Shaak Ti and she requested that Fives be sent to Coruscant to undergo further tests and to plead his case to the Supreme Chancellor. While en route to Coruscant, Nala Se secretly injected Fives, barely conscious due to the stress of his impromptu surgery, with a form of mind clouding drug.

Although he tried to explain his actions, Fox was summarily executed for failing to provide the clones with Vader's description. The Dark Lord of the Sith used telekinesis to break the clone's neck, ending the life of Commander Fox.
Fives' last words are "This is the end. Forget the mission. Oh, nightmare. I'm free." Deeply dark and ironic words for an audience that knows Order 66 is still coming.
She hired the bounty hunter Fennec Shand to retrieve Omega and return her safely, but when Lama Su declared that he wanted her to terminate the young clone, Nala Se called off Shand, believing Omega would be safe as long as Lama Su did not have her and agreed to pay the bounty hunter.
