Jake's story picks up fifteen years after the Battle of the Hallelujah Mountains and the expulsion of 90% of the humans on Pandora, and Jake is now living as a full Na'vi, his consciousness permanently fused into his avatar body. In many ways, Jake is not the same person we left.

Well, according to the original script of Avatar, Jake was 22 when he blasted off to Pandora. When he arrived, 6 years had passed while he was in cryo sleep, so his human body was 29 when it died, since he was transferred to his Avatar on his birthday.
