The Smile Entity, also called The Monstrosity or simply The Smile Demon, is the main antagonist of the 2020 horror short film Laura Hasn't Slept, as well as its 2022 film adaptation/sequel Smile.
Northcott (Robin Weigert) reveals herself to be the demon. Her own therapist attacks her and shatters the sacred trust between doctor and patient — it's a shocking moment that represents the ultimate betrayal. Ironically, this helps Rose realize that she may have betrayed her own mother in her darkest time of need.
However, it goes deeper than being an 'evil creature', a curse, or a demon. The entity is something like Cthulhu. Hear me out; in the end of the film, when the entity's true form is revealed, we see Rose totally shut down. She stares, unable to comprehend, as rows of teeth and raw flesh is exposed.
The Smile demon's true form is the stuff that nightmares are made of. When Rose finally sees it at the end of the movie, she is stunned into silence. The entity in Smile is able to shapeshift and become any person that it wants to.

What is the beast in Smile ending?
In the end, Rose goes to her childhood home to confront her unresolved trauma regarding her mother's death, in hopes of defeating the monster. There, she fights a giant, deformed version of her mom and sets the house on fire, before driving to her ex Joel's house (Kyle Gallner) for safety.
Smile featured the traumatizing death of Rose's cat. The movie never reveals how the pet died, but there may be an answer for what happened to it. One of the most heart-breaking casualties in Smile was Rose's cat, Mustache, but the film never revealed what actually happened to it.
It is a demonic entity responsible for an ongoing series of gruesome murders, which it facilitates through a “curse” which passes from victim to victim in a “chain” stretching back to an unknown beginning.

The monster in Smile passes from person to person like a virus, needing new victims in order to survive. As such, one possible course of action would be to remove oneself from society entirely, escaping to a place of isolation where there are no other human beings nearby to infect.
Ok so the movie Smile is based around a curse/entity that feeds and spreads through trauma. The main character Rose gets the Trauma both from her mother's death when she is young and also seeing her patient who had the Trauma die violently.
Most of the pictures are incredibly gory and bloody. May upset some. A woman accidentally bites her thumb creating a cut, some blood is shown. Blood and gore is infrequent but when it happens there is lots of it.

Instead the box contains the bloody body of Mustache. Rose cradles the dead cat (thankfully a fake or CGI) and begs the party goers to believe she had nothing to do with the cat's demise or placement in the present. The animal wrangler on the film was Diana Ventura.
She pours it on herself and lights a match. After spending our time seeing things from Rose's perspective, often in uncomfortably closeup shots, we see her final moments through Joel's eyes, who can't look away despite his horror. The curse has now been passed on to him.
In the end, Smile has all three of those endings. Rose brutally stabs a terrified patient to death at her hospital in front of her screaming boss, Morgan (Kal Penn). But that turns out to be a dream she's having while passed out in her car in front of the hospital, and she flees the hospital and Morgan in horror.
