The former Jedi Padawan and master of Ezra Bridger lost his sight in a duel with Darth Maul on Malachor, resulting in him having to deepen his bond with the Force to make sense of the galaxy around him once again.
But once Maul made his true intentions known, that he intended to take Ezra away from him, he struck Kanan with his saber, blinding him, and would have killed him had Ahsoka not defended him. Kanan found an old Temple Guard mask to cover his ruined eyes and stood up to face the deceiver again.
During a mission to Malachor, Jarrus was blinded by the former Sith Lord Maul. After months of retreat, Jarrus restored his connection to the Force and repaired his frayed relationship with Bridger with the help of a mysterious being called Bendu, who taught him to see things through the Force.
After he was permanently blinded by Maul, Kanan was forced to rely on this power to sense his surroundings until the very end of his life, though his eyes were restored by the Force shortly before his death.

Why does Kanan look different?
After being blinded by Maul on Malachor, Kanan's eyes lose their teal colors, and became pale, even light blue. During this time, he now grows beard. Despite his wounds however, he now wears a mask that he has gotten in Malachor.
He wasn't healed, he was immersed in the Force. We saw Kanan grow as a Jedi and learn to accept the will and the aid of the Force to navigate his world while physically blind.
Kanan had not let his blindness hold him back, but the Force allowed him a final gift as he sacrificed his life. In a beautiful twist of fate, the Force allowed Kanan to see again for a final moment, giving him one last look at Hera, the woman he loved.
According to Wookiepedia and Star Wars Rebels the Visual Guide, Caleb Dume/Kanan Jarrus was born in the year 33 BBY (Before the Battle of Yavin). We know that Order 66 took place in 19 BBY, so that means Kanan had to be around 14 years old during Order 66.

Ezra confronted Thrawn and engaged in combat with the Grand Admiral before summoning multiple Purrgil, space whales that can travel through hyperspace. The Purrgil captured the ship Ezra and Thrawn were in and jumped to hyperspace, leaving no track behind.

In season 4, They admitted their feelings for each other in season 4 and entered a romantic relationship. Later, in the episode “Jedi Night”, Kanan gave his life to save her, Sabine and Ezra and looked at her one last time. Doing so not only protected Hera, but also their unborn child as well.
Dume was a mysterious and abnormally large male Loth-wolf who shared the birth name of Jedi Knight Kanan Jarrus—Dume. Following the death of Kanan during a mission to rescue Hera Syndulla, "Dume" became the continuation of Jarrus' will, communing with Jarrus' apprentice Ezra Bridger.
In the Kanan comic, Caleb cut off his Padawan braid and let his hair grow so that he could become Kanan, the next stage in his life. Now, Kanan cuts away his hair so that he can progress to the next stage of his life, possibly becoming Caleb once again. ... or acknowledging the man who could/should have died back then.
