When the bomb goes off, Michael is irked to discover that his job wasn't completed after all, and someone like Tommy doesn't die quite that easily. After one final speech-off, Tommy shoots him in the eye, ending Polly's prophecy that there'll be a war, and one of them will die.
Michael told Tommy that Polly's death was his fault, to which Tommy said Polly still visited him in his dreams. "She'll visit me no more," Tommy said as he pulled the trigger and shot Michael through the eye. Tommy then retreated into nature, planning to take his own life before illness took him.
After the Changrettas are taken down, Michael returns to meet with Tommy, Polly, and Ada. Despite the plan being a ruse, Tommy is upset with Michael for not warning him about it, since as far as he knew, he was letting Tommy die.
Tommy shoots Alfie dead at the end of a long feud between the two, during which Solomons sells Shelby out to the Italians. Solomons also claims that he's dying of cancer, and once Alfie shoots Tommy in the side to goad him into putting him out of his misery, Shelby has no real reason to keep him alive.
The scene shows that Polly chooses to take Michael's side in the conflict before she dies. The use of Michael's blood also implies that Polly set the curse upon Tommy, as she has previously claimed a connection to the supernatural, seemingly communing with spirits.

Elizabeth Pollyanna “Polly” Gray (née Shelby) is the matriarch of the Shelby Family and sister to Arthur Shelby Sr. She is also the mother of Michael and Anna Gray, and aunt of Thomas, Arthur, John, Finn and Ada Shelby.
