There, Batman learns that the unstable properties of the Titan formula are mutating in Joker's blood, gradually killing him. Joker captures Batman and performs a blood transfusion on him, infecting him with the same fatal disease. Joker also reveals that Gotham hospitals have been poisoned with his infected blood.
A flashback reveals that before the Joker died, his infected blood was used in blood transfusions, infecting five people including Batman; Batman, concealing his infection from others, imprisoned the four other recipients who were physically and mentally transforming into the Joker.
It was never fully revealed what the exact cause for the creation of the infection was other than it originating from Joker's mutated blood. It was probable that the chemicals that caused the Titan Disease (and maybe chemicals already in the clown's blood) caused Joker's blood to mutate, creating the infection.
Carrying Joker out and dropping him on the car is symbolic of the end of Joker's reign of terror. It's not a lost Batman emotionally carrying a man he respected, because he didn't respect him!
In an alternate universe that exists within DC's newly found Dark Multiverse, the Joker infects Batman with a modified Joker toxin, which slowly turns him into a version of the Joker. Thanks to Joker's toxins, Batman's personality and perception become warped in terrifyingly destructive ways.
