Sadly, a combination of pandemic timing issues and the actor wanting to leave his legacy in the series kept this from happening. In regard to the latter, not having Bradley in any capacity does wholly separate this film from its predecessors.

The original book that it's based on, The Hellbound Heart, was suggestive of a female Pinhead, and the comic books also explored the idea. There's also plenty of fan art, wonderful works that all theorized the possibility of Pinhead as a woman. So it only felt appropriate to us.
In The Hellbound Heart, Cenobites are described as androgynous, and Pinhead is said to have a feminine-sounding voice, which was used as inspiration to cast Clayton as the character.

Director David Bruckner has a good reason for casting Jamie Clayton as Pinhead for his reboot of the Clive Barker adaptation Hellraiser. Pinhead isn't Pinhead.
The actor added that he had no further thoughts on Clayton herself beyond admitting that it was "an interesting casting decision." However, Bradley continued to talk about the decision to turn Pinhead female, even taking time to point out, with a chuckle, that his Pinhead did wear a skirt.
While Barker worked on writing with his friend Peter Atkins (script-writer for several Hellraiser films), Bradley started acting. Bradley made it to the Movie Monster Hall Of Fame with his role as the cenobite, Pinhead, who he portrayed in eight of the Hellraiser movies.

In the final scene, Voight's body is ripped and torn into new shapes so that he resembles a Cenobite himself. Of course, there are many ways that this could be interpreted. In the mythos, only those who are so obsessed with solving a puzzle that can open a gateway (or 'Schism') can be considered. Those who have a taste for the outermost reaches of human experience and have no moral limitations for what they will do to achieve those desires would even be considered to become a Cenobite.
At the end of 2022's Hellraiser, Riley chooses the Lament Configuration over the Lazarus Configuration, but whether she makes the right decision is left open to interpretation.
