Skyfall was confirmed as the title at a press conference on 3 November 2011, during which co-producer Barbara Broccoli said that the title "has some emotional context which will be revealed in the film". The title refers to the name of Bond's childhood home, "Skyfall", and the setting for the film's finale.
When Bond first hears the word 'Skyfall', it's an attempt by the MI6 psychologist to test him. Bond refuses to answer the question which essentially provides the answer itself. There are clearly some unresolved issues from his childhood and a traumatic event that he needs to address.
Thus, the psychiatrist deliberately used “Skyfall” to guage the degree to which reminding him of his childhood/loss of his parents would trigger him. He said, “Done.”, for two reasons. First, because it did trigger him, and he wasn't about to let them into that part of his mind and heart.

To play the part of Kincade, Mendes cast Albert Finney. The producers briefly considered approaching Sean Connery to play the role in a nod to the 50th anniversary of the series, but decided not to as they felt Connery's presence would have been seen as stunt casting and might disengage the audience.

I think when Bond introduces her to Kincade as “M”, Kincade must've heard this as “Em” which his why he then followed up with Emma. I think the name Olivia turns up in Spectre when Bond is watching the footage stored inside the bulldog ornament, her name is on the outside of the box her personal items were stored in.
Killing off Judi Dench's M in Skyfall was a bold move to revitalize the 007 franchise after Quantum of Solace's poor reception. Dench's departure was part of a plan to give Bond a new world with a new M, introducing major changes after the problematic previous film.
Regrettably, the iconic Skyfall house depicted in the film was a product of movie magic and constructed as a set in Surrey, England, situated on Department of Defence land. Why was Skyfall (2012 movie) such a huge box office hit, compared to the other James Bond movies? Skyfall was a pathbreaker. There is a gritty, real-world feel to Skyfall that makes good use of Craig's down-to-earthiness. It suits well to the present world.
Unfortunately, Connery's advanced age meant that any plans had to be shelved - he is 85, after all - and the scenes were rewritten and quietly swept under the carpet.
