An outstanding film with incredible acting but it's very important not to let any kid under 14 watch it. This film is a psychological horror about a man with DID (disassociated identity disorder) who abducts 3 teenage girls. It is very disturbing especially for kids who don't understand mental disorders.
Extremely gory near the end with guts and results of shotgun blasts are seen. A young girl points a shotgun to a man. There is a offscreen act of cannibalism. We see the aftermath of one victim briefly, with her intestines visible.
The R-rating doesn't ban children under 17 from a movie, it restricts them to watching it with a parent or guardian (and includes a recommendation that parents and guardians learn more before allowing their child to see the movie).

Kevin's more evil personalities, “Dennis” and “Patricia” are awaiting the arrival of “The Beast” which is Kevin's 24th personality and is an animal-like killer who seeks to rid the world of the “impure,” which are those who have not suffered.
