The character Hope van Dyne first appeared in Ant-Man (2015). Van Dyne is introduced in Ant-Man as the daughter of Hank Pym and Janet van Dyne and a senior board member of Pym Technologies who helps Darren Cross take over the company. Throughout the film, character progression brings Hope closer to becoming a hero.
Created by David Michelinie and John Byrne, the character first appeared in Marvel Premiere #47 (April 1979). Cassandra Lang is the daughter of the superhero Scott Lang / Ant-Man. She has also been known as Stature, Stinger, and Ant-Girl at various points in her history.
Granted, Hope (Lilly) isn't referred to as Cassie's (Kathryn Newton) stepmom, and Hope and Scott (Paul Rudd) aren't officially married, as far as we know.

She's now in her teenage years and following in her father's footsteps by harnessing size-changing technology and becoming a member of Ant-Man's superhero team, along with Scott Lang himself and Hope Van Dyne as the Wasp.
Following the events of the Civil War, Scott had not been in contact with Pym and Hope for a while, and his relationship with the latter ended as well. Scott was under house arrest, and the tech of Ant-Man belonged to Hope and Pym, so they had to become fugitives.

Prior to his appearance in Quantumania, though, Majors made his MCU debut in the Season 1 finale of Loki, playing one (of many) variants of his villain Kang the Conqueror named He Who Remains.
It's never specifically stated but our best guess is that Janet simply felt guilty about choosing her family over all of the people she left subjugated to Kang in the Quantum Realm. If she actually acknowledged it, that would've meant having to deal with it, and she just wanted to ignore it.
He was exiled to the realm by the Council of Kangs — a group made up of other versions of the villain from throughout the multiverse. Quantum Kang was banished because he wanted to slaughter the council so that he was the only version to exist.
