Though we never see Seven's assimilation, we know that unlike adults who are assimilated, she was placed in a maturation chamber where her synaptic pathways were remapped. Seven of Nine returned to Earth with everyone aboard the USS Voyager at the end of Star Trek: Voyager. After that is 100% unknown in Established Star Trek Canon and will have to be answered in Star Trek: Picard since she will be in there.
The two were planning their fourth date in the series finale (“Endgame”) and future Admiral Janeway revealed they did get married in what became an alternate timeline. In that future, Seven was killed on a mission in 2381 and Chakotay died in 2394.

Why is Janeway not in Picard?
Admiral Janeway cameo considered for Picard Season 3, but showrunner Terry Matalas feared it would overshadow Seven of Nine's story. Seven of Nine's journey in Picard Season 3 was one of the best story arcs, starting as First Officer and ending as Captain of the Enterprise-G.
A Chakotay cameo would, therefore, have distracted from the overall story of Admiral Jean-Luc Picard (Patrick Stewart) confronting the trauma of his past. Chakotay being at the head of a fascist government also doesn't fit with his character, as he was a member of the Maquis, which fought against Cardassian oppression.
By the time of the series finale, "Endgame", The Doctor had managed to remove the implant, allowing Seven to pursue a relationship with Chakotay. The alternative future seen at the start of the episode showed that Seven and Chakotay were eventually married, but she died while Voyager was still travelling home.
Kate Mulgrew prevented Janeway and Chakotay from becoming a couple on Star Trek: Voyager, prioritizing Janeway's likability and reputation. A potential romance between Janeway and Chakotay was also hindered by Starfleet rules, with Janeway focusing on her task of getting Voyager home.
