It means that during the arena, during his killing spree, he realizes that there's more to life than killing, and he's just a pawn in the whole scheme of things. He realizes that he's a cruel person the capitol created, and he wouldn't mind dying because he wasn't alive on the inside anyways.

In the books, Collins never declares when Panem is created in the wake of a global war spurred by a series of environmental disasters. An early draft of The Hunger Games script alludes to it happening about 300 years from our current future, so that's at least a window to work with.
In the books, her last arrow is used to secure the tourniquet to stop the bleeding of Peetas leg, and one of the reasons why she does not shoot Cato sooner to end his suffering is that she fears removing it would make Peeta bleed to death. This is just an oversight of the movie.
