Many decades have passed since the events of "Obsidian." Through a hallucination, it is revealed that Jake has passed away, which severely traumatized Finn. In this hallucination, Finn goes on an adventure that takes him back to his pre-teens, but things seem off.
During Adventure Time, it was revealed that Finn and Jake were the same age, and the series ended when Finn was 17. Therefore, it would only make sense that Jake would pass away by the time Finn reached adulthood.
The Adventure Time: Distant Lands special "Together Again" takes place sometime after the end of the series, when Finn has died of old age. He then journeys through the Dead Worlds to be reunited with Jake, who predeceased him.
