The grief monster rips off its skin, revealing its horrible, true form, then snaps open Rose's mouth, crawling inside of her. Fully possessed by the demon, Rose douses herself in gasoline and sets herself on fire — just as the real Joel bursts inside her home and witnesses her death, becoming the next victim.
How Rose's Death Sets Up Smile's Sequel. Not only is Rose unable to defeat the demon in Smile's ending but, in a truly vicious twist, the entity waits until Joel arrives before possessing and killing her. This means that it will be able to live on in him since the curse needs a witness to pass itself on.
Rose sees the monster, has a mental breakdown, and can't move. She truly and fully embodies the curse by starting a fire with her own body. Joel realizes too late that Rose has given him the curse. It's an effective ending since it doesn't seem that Rose would be able to survive this.
Ok so the movie Smile is based around a curse/entity that feeds and spreads through trauma. The main character Rose gets the Trauma both from her mother's death when she is young and also seeing her patient who had the Trauma die violently.
Smile clearly wants the audience to see the connection between Rose's curse and mental illness. Throughout the movie, characters use therapy buzzwords and often suggest that Rose's mental state is a result of her genetics and past trauma.
We learn that the young Rose allowed her mother to die rather than call for help because she was both afraid and angry with her mother. The demon believes this long-repressed revelation will paralyze Rose with guilt, making the task of subduing and overtaking her easier.
In the end, Smile has all three of those endings. Rose brutally stabs a terrified patient to death at her hospital in front of her screaming boss, Morgan (Kal Penn). But that turns out to be a dream she's having while passed out in her car in front of the hospital, and she flees the hospital and Morgan in horror.

One infected person is consumed, the entity forces them to take their own life and then the witness becomes cursed. But, what if multiple people witness that death?

So, the entity uses Rose's body to pour gasoline all over the psychiatrist, lighting a match just as Joel comes through the door. Rose's body is smiling while she burns away, and we can see in Joel's eyes he will be scarred for life by that disturbing suicide.
The Smile movie explained the entity needs another person to witness her death to carry on its curse, Rose secludes herself in her childhood home. There, the entity takes the form of her mother, revealing that, after overdosing, she asked a then 10-year-old Rose to call for help. Rose declined and her mother died.
There's obviously a reason why the smile demon appears as a giant, looming depiction of Rose's Mom during the final act. One of the most unsettling moments in "Smile" takes place just before that when Rose's trusted therapist, Dr. Northcott (Robin Weigert) reveals herself to be the demon.
Joel arrives in time to see Rose pouring gasoline on herself and then lighting a match. The chilling final shot of Smile zooms into Joel's eye as he watches her burning to death. With Rose dying in a horrifying way, it continues the cycle and secures Joel as its next victim.
Rose begins experiencing hallucinations induced by the demon. Shortly after, she has trouble finding her cat, Mustache, and its tragic fate is revealed during her nephew's birthday party. The boy opens Rose's present to discover her cat's dead body.
In the end, we see the young Rose returning to the Titanic once more, where she is greeted by Jack and the other passengers who lost their lives with the sinking of the ship, suggesting that either she passed away and has returned to him in spirit, or that she's dreaming of seeing him again.
It is a sadistic and violent demonic entity that feeds off of the trauma of its victims, tormenting them to sustain itself, but also because it enjoys causing its victims to suffer, until they eventually kill themselves, or until it possesses them and forces them to kill themselves.
Most of the pictures are incredibly gory and bloody. May upset some. A woman accidentally bites her thumb creating a cut, some blood is shown. Blood and gore is infrequent but when it happens there is lots of it.
She pours it on herself and lights a match. After spending our time seeing things from Rose's perspective, often in uncomfortably closeup shots, we see her final moments through Joel's eyes, who can't look away despite his horror. The curse has now been passed on to him.
With the colors, we wanted to do something unusual. We had read this study that was from back in the sixties where they found out that this sort of Pepto Bismol pink color somehow was supposed to make violent inmates less violent. So my production designer, Lester Cohen, and I, really grabbed onto that.
