Laura (Anna-Maria Sieklucka) survived the tunnel attack at the end of the first movie, but she lost her unborn baby. Her mob-boss-kidnapper-turned-fiancé Massimo (Michele Morrone) doesn't know that Laura was ever pregnant. The two get married and have a lot of sex on their honeymoon.
What happened to Laura at the end of the first 365 Days?
The way the final shots were filmed definitely seem to suggest that Laura has been killed. But the film is based on the first of three books, and Laura survives in those. The second book confirms that Laura manages to survive the assassination attempt.
In the end, Laura runs away with Nacho and becomes pregnant with his child. With disturbing twist after twist, Kolejne 365 dni subverts the notion that 365 Dni series is a love story at all (because it's not). While Massimo is framed as a romantic lead, his behavior in Kolejne 365 dni is a reminder of his rotten core.
At his villa, Massimo reveals to Laura who he was and his story. He tells her that when he was injured, all he could think about was her. After searching for years and finally spotting her, he kidnapped her, intending to keep her as a prisoner for 365 days in the hopes that she will fall in love with him.
The book ends with Laura choosing to be with Nacho, but that is after Massimo holds her against her will, drugs her and tries to get her pregnant. It's not really a surprise she escaped with Nacho in that scenario. The epilogue shows they have a daughter together, while Massimo has given up and agreed to a divorce.

Does Nacho really love Laura?
While on a business trip to Portugal, Laura meets Nacho and the two end up getting together and having sex on the beach. Nacho confesses his love for her, and says he wants to be with her but he won't force her to be with him.
Massimo is still a mafia boss and still guilty of kidnapping, coercion, and a host of other criminal charges, but viewers can rest assured that he is no cheater.
Laura has lost her baby due to the accident that took place in the final scenes of the previous movie. They both are happy until Laura starts feeling bored as she has nothing to do while Massimo is busy with his work. Daily arguments force her mind to get devoted to Nacho, Massimo's gardener.
While the movie deviated a bit from the second book (especially with the character of Nacho), it ended in the same way with Laura shot. However, a big difference was that she was still pregnant with Massimo's baby. At the start of the third book, Massimo is faced with the decision of saving Laura or their unborn child.
When Laura discovers Massimo is the one who killed her dog, it is not long before she seduces him, escapes, and runs off with Nacho. Surprisingly, Massimo then assents to divorce, meaning Laura and Nacho end up living happily ever after and the unhinged mob boss simply accepts that his former paramour is gone for good.
Laura eventually gets away with Nacho, they get engaged and in an epilogue, it's revealed that they have a daughter together. Massimo has given up on Laura and divorced her, letting her keep the company too. It's a happy ending of sorts, but there's a lot of icky dark stuff that has to happen along the way.
Team Nacho - He is more romantic, he cooks and takes care of her. He wants to show her the world. He shares his hobbies with her. Massimo is controlling, cuts her off and doesn't answer her questions, but demands that she answer his.
There was an intimate coordinator during the scene, the sex is choreographed and most important of all the actors used prosthetics/modesty garments/pillows and other tools to avoid/stop genital contact (and to create an illusion of sex). This is how sex is simulated in movies.
When Laura catches Adriano and Anna hooking up, she believes she's catching Massimo red-handed. She spends a short period of time feeling heartbroken and devastated by what she believes she saw. It turns out Massimo never cheated on her, and it was simply a case of mistaken identity.
Basically, Massimo (Michele Morrone) becomes obsessed with Laura (Anna-Maria Sieklucka) after seeing her on the beach on the same day that his father was killed by a rival gang. Massimo himself was injured in the attack, and Laura's face was the last thing he remembers seeing before losing consciousness.
Laura kisses Nacho. He then confesses his love for her and drives her to a random house that may or may not be his. They have a dramatic conversation on the beach that quickly leads to sex…on the beach.
