After the premise of Old is revealed that the characters vacationing on a beach are aging quickly (one year every half an hour), a character says that "trial 73" is over. This is the big twist that is always part of an M. Night Shyamalan movie, including The Visit.
Kara and Trent continue to age into their late teens, and after having sex with each other, Kara gets pregnant at a rapid pace. She gives birth, but the baby dies due to the time rapidly passing. The attempts to leave grow increasingly tense as Charles's worsening schizophrenia causes him to kill Brendan.
Trent and Maddox are flown to the airport on a helicopter, where they will meet with their aunt, and live out the rest of their lives having skipped four decades of their life. It's not exactly a happy ending, but at least now they can prevent anyone else from going to the beach that makes you old.

Why did Kara have a baby in Old?
Trapped on the beach, children Trent, Maddox, and Kara are subjected to years' worth of hormones, growing pains, and a series of firsts in the span of hours. Teenagers Trent and Kara act on their new feelings, resulting in Kara's accelerated pregnancy and subsequent childbirth.
Though Shyamalan's Old doesn't explain it, hemophilia reduces people's ability to make blood clots, which is what stops an area from bleeding. Because of his disease and the beach's timeline continually enabling easy bleeds, his nose never stops bleeding.
The coral, for reasons unexplained, counteracted the effects of the cove's rocks. Once Maddox and Trent were able to swim through the coral, they were free. The damage to their lives, however, had already been done and couldn't be reversed.
The plot of the movie Old implies that the accelerated aging can be caused by a natural environment – rocks with specific rare minerals. These rocks were radiating harmful magnetic fields or some other waves that caused accelerated aging.
