Tony stark created a mapping system so they could find their way around the quantum realm, time is extremely abstract down there so 5 years could be 1 second or 80 years depending on where you would be.

Although it might seem that Ant-Man could have found Kang in the Quantum Realm during Avengers: Endgame, it is important to remember that Scott Lang thought he was trapped for only five hours. He was unaffected because the Quantum-realm is describe as a universe within a universe in the first two ant man films. The Snap dusted half of all life in the universe, ergo other universes or alternate timelines are unaffected, which tracks.
Ant-Man is also pretty powerful, as his strength remains high in proportion to his size and he can pack a mean punch when no one sees it coming. However, at the end of the day, Scott Lang is still not that experienced, and that means that, even with a super-powered suit, he doesn't match up with most of the Avengers.
The theory posits Ego, Odin, and The Ancient One as three of Thanos' greatest fears, and he waited until all of them were dead to make his move. This would make sense, as all of these characters were massively powerful and had strong ties to the Avengers, which meant they could be called in for backup at any time.
