The Smile Entity, also called The Monstrosity or simply The Smile Demon, is the main antagonist of the 2020 horror short film Laura Hasn't Slept, as well as its 2022 film adaptation/sequel Smile.
In reality, Joel has tracked Rose's phone to her old house and finds her outside. Rose panics and runs back inside, where the demon reveals its true form – a skinless, semi-humanoid monstrosity with multiple sets of malformed jaws nesting within an enormous, smiling mouth.
The monster in Smile passes from person to person like a virus, needing new victims in order to survive. As such, one possible course of action would be to remove oneself from society entirely, escaping to a place of isolation where there are no other human beings nearby to infect.

One infected person is consumed, the entity forces them to take their own life and then the witness becomes cursed. But, what if multiple people witness that death?

Why didn t Rose win in Smile?
However, in Smile, Rose never gets any reprieve from the curse. The entity hounds her relentlessly and with a gleefully sadistic streak, and it is dead-set on eventually overpowering her. Rose's mistake, ultimately, is that she keeps running and fighting before eventually giving up.
The final image of the film is Rose's burning body reflected in the iris of Joel's eye, burning itself onto his psyche forever. Even though Rose did everything she was supposed to do — that we're supposed to do to process our own trauma — the monster still won. The cycle will continue.
The release date for Smile 2 is October 18, 2024, approximately two years after the release of the original film. While it is uncertain whether the sequel will follow the original protagonist or introduce a new character, there is speculation about who will be battling the demonic entity in Smile 2.
The people around Rose, including Trevor, her therapist Dr. Northcott (Robin Weigert), her boss Dr. Desai (Kal Penn), and her sister Holly (Gillian Zinzer), certainly seem to think the problem is more neurochemical than supernatural—that is, until it's way too late.
With the curse seemingly over, she visits Joel in the Smile ending to apologize for pushing him out of her life. In what could be considered one of Smile's best reveals, however, Joel turns out to be the creature in disguise, revealing that Rose never actually left the house and is instead trapped in her own mind.
Smile,directed by Parker Finn, chronicles a set stories like these. Before going to the theatres, I'd recommend you take the trigger warnings surrounding suicide and post-traumatic stress disorder (PTSD) very seriously.
The visions inspire Rose to look into Gabriel Munoz's suicide and she learns Gabriel was also smiling strangely immediately before he died. Rose checks on her patient Carl Ranken, who was previously admitted to the hospital for a manic episode.
Rose begins experiencing hallucinations induced by the demon. Shortly after, she has trouble finding her cat, Mustache, and its tragic fate is revealed during her nephew's birthday party. The boy opens Rose's present to discover her cat's dead body.
The grief monster rips off its skin, revealing its horrible, true form, then snaps open Rose's mouth, crawling inside of her. Fully possessed by the demon, Rose douses herself in gasoline and sets herself on fire — just as the real Joel bursts inside her home and witnesses her death, becoming the next victim.
She pours it on herself and lights a match. After spending our time seeing things from Rose's perspective, often in uncomfortably closeup shots, we see her final moments through Joel's eyes, who can't look away despite his horror. The curse has now been passed on to him.

Ok so the movie Smile is based around a curse/entity that feeds and spreads through trauma. The main character Rose gets the Trauma both from her mother's death when she is young and also seeing her patient who had the Trauma die violently.

Very scary horror. This is an extremely scary, graphic horror movie that should not be taken lightly. I have seen my fair share of horror movies over the years but this one definitely has a different feel to it. It is chilling, unnerving, and very difficult to watch sometimes.

In the end, Smile has all three of those endings. Rose brutally stabs a terrified patient to death at her hospital in front of her screaming boss, Morgan (Kal Penn). But that turns out to be a dream she's having while passed out in her car in front of the hospital, and she flees the hospital and Morgan in horror.
