Kay is increasingly repulsed by Michael's criminal life, particularly the fact that Michael's button men have become Anthony's playmates. She also fears that if the baby turns out to be another son, it will tie them to the Mafia forever. During Michael's trip to Cuba, Kay decides to leave Michael for good.
At first, Kay's rebellion is silent and private: she aborts the child she is carrying. But later, when she tells Michael that she is leaving him and reveals the truth that her “miscarriage” was really an abortion, she challenges him in a way no one ever has before.
Meanwhile, Kay decides to leave Michael and take their children with her, believing Michael will always live in a world of crime and violence. Michael asks her to reconsider, but Kay reveals she aborted their unborn son because she refused to bring another of his children into the world.

Did Michael actually love Kay?
Al Pacino said that Michael loved Kay from the beginning. Remember that Mario's character of Michael was a man who had dreams and I do not ever want to dismiss this important deep aspect of Michael.
Kay did not know. In the Movie, Michael tracks Kay down after he has been back for several years. In the movie Kay does not know. In the book Kay does not know.
She still loved Michael. She called Mama Corleone and came over to his place. Michael mentioned he was only glad to see Kay and his father out of his entire family. He also confessed to her he slept with another woman in Sicily!
