In the end, Rose goes to her childhood home to confront her unresolved trauma regarding her mother's death, in hopes of defeating the monster. There, she fights a giant, deformed version of her mom and sets the house on fire, before driving to her ex Joel's house (Kyle Gallner) for safety.
We learn that the young Rose allowed her mother to die rather than call for help because she was both afraid and angry with her mother. The demon believes this long-repressed revelation will paralyze Rose with guilt, making the task of subduing and overtaking her easier.
The concept of someone going through the most harrowing events, with a smile plastered on their face, makes for a great metaphor on the lives of patients suffering from mental illness, which is often invisible to the outside world.

What was Rose's trauma in Smile?
Ok so the movie Smile is based around a curse/entity that feeds and spreads through trauma. The main character Rose gets the Trauma both from her mother's death when she is young and also seeing her patient who had the Trauma die violently.
She pours it on herself and lights a match. After spending our time seeing things from Rose's perspective, often in uncomfortably closeup shots, we see her final moments through Joel's eyes, who can't look away despite his horror. The curse has now been passed on to him.

Cristobal Tapia de Veer's musical score uses many electronic, ambient sounds acting as its own character to heighten the anxiety of most scenes. The cinematography of Mike Gioulakis often goes upside down in wide shots to chronicle the distorted point of view Rose is viewing things from.
Smile clearly wants the audience to see the connection between Rose's curse and mental illness. Throughout the movie, characters use therapy buzzwords and often suggest that Rose's mental state is a result of her genetics and past trauma.
What happened to Rose's father? The film's opening sequence pans across a series of portraits of Rose's family, with her mother, father, and her sister Holly all happy together. Then Rose's father disappears from the pictures. It's unclear whether he died or abandoned the family.
What Is The Monster In Smile? The monster in Smile is a humanoid creature who has a huge grin, many jaws, and no skin. The monster essentially eats people and then those people become part of their body.
Rose forgives herself at that moment, but that doesn't deter the creature, which transforms into a monstrous version of Rose's mother. She sets the creature on fire and runs out of the house. With the curse seemingly over, she visits Joel in the Smile ending to apologize for pushing him out of her life.
Joel arrives in time to see Rose pouring gasoline on herself and then lighting a match. The chilling final shot of Smile zooms into Joel's eye as he watches her burning to death. With Rose dying in a horrifying way, it continues the cycle and secures Joel as its next victim. Sorry, Joel.
While its MO was for Rose to harm herself, the demon was also capable of physically interacting with the real world, as shown by the twist of it impersonating Dr. Madeline and attacking Rose. Thus, the monster could have feasibly killed Rose's cat.

We had read this study that was from back in the sixties where they found out that this sort of Pepto Bismol pink color somehow was supposed to make violent inmates less violent.
