A vacationing family discovers that the secluded beach where they're relaxing for a few hours is somehow causing them to age rapidly, reducing their entire lives into a single day.
The plot of the movie Old implies that the accelerated aging can be caused by a natural environment – rocks with specific rare minerals. These rocks were radiating harmful magnetic fields or some other waves that caused accelerated aging.
So what is actually going on with the beach? Electromagnetic material surrounding the beach is causing cells to age incredibly quickly – at the rate of around a year every half an hour. The kids are still growing so their aging is more obvious than the adult characters.
Kara and Trent continue to age into their late teens, and after having sex with each other, Kara gets pregnant at a rapid pace. She gives birth, but the baby dies due to the time rapidly passing. The attempts to leave grow increasingly tense as Charles's worsening schizophrenia causes him to kill Brendan.
So what's the twist? As it turns out, the beach is being used by a pharmaceutical company so that they can test the efficacy of their drugs over the course of a single day rather than a patient's normal lifetime.

Why was the coral safe in Old?
The coral, for reasons unexplained, counteracted the effects of the cove's rocks. Once Maddox and Trent were able to swim through the coral, they were free. The damage to their lives, however, had already been done and couldn't be reversed.
It is soon revealed that most of the beachgoers have some kind of medical affliction, and while Mid-Size Sedan doesn't spell out his condition, he appears to be afflicted with hemophilia.
Teenagers Trent and Kara act on their new feelings, resulting in Kara's accelerated pregnancy and subsequent childbirth. Because the group determines that they are all aging two years every hour, the moments that follow the baby's birth amount to weeks without sustenance, and the child quickly dies of starvation.
Importantly, each family has at least one person with a pre-existing condition. Think multiple sclerosis, hypocalcemia, blood clotting disorder, mental illness, and epilepsy, for starters. Key to the storyline too is that no one can leave this oceanfront resort.
In M. Night Shyamalan's Old, there are three intersecting mysteries. The first mystery is why is a specific beach is causing people who inhabit it to age at a highly accelerated rate. The second mystery is why do people blackout and end up back on the beach whenever they try to leave.

While it received critical acclaim, twist master Shyamalan might just be its biggest fan, claiming that “from the moment I read this, I was changed.” It's easy to see why Sandcastle appealed so much to the director of The Sixth Sense, as it combines psychological and supernatural horror.
That may be the point of the fictional rap star, whose given name isn't revealed to his fellow beach goers and the audience until well into the movie. For actor Pierre, who says Shyamalan “pre-determined” that stage name, Mid-Size is a conversation about duality and a chance to examine our own snap judgments.

During Becca and Tyler's final night at the farm, the truth was unveiled: Nana and Pop Pop were patients at the mental hospital where Becca and Tyler's grandparents volunteered, and the real ones were murdered by them and their bodies kept in the basement.
