The Doctor and River escape to the top of the pyramid followed by Amy and Rory. The Doctor marries River on the spot and whispers to her to look into his eye. There she sees the miniaturized Doctor and realizes that he is inside the Teselecta ship and will not actually die if time's flow is restored.
Clara is the future child of the Doctor and River who's had her memory erased. The two Time Lords must be up to something on their nights away from River's prison cell.
While those years occur off-screen, it is heavily implied that The Doctor remained on Darillium with River the entire time. The Doctor loved River Song enough that he suppressed the urge to roam for over two decades. This is something he was never able to do before River, and has never done again since.
River doesn't recognise the Twelfth Doctor as the Doctor because "he has limits", referring to the Time Lords' twelve-regenerations limit first mentioned in The Deadly Assassin (1976).

She does, however, meet Peter Capaldi's Twelfth Doctor in Christmas episode, "The Husbands of River Song," where the couple spend 24 years together. Finally, River heads to the library where she'll partake in her last adventure, meeting the Tenth Doctor and fighting the Vashta Nerada, before sacrificing herself.

In these episodes, River Song interacts with the Tenth Doctor (David Tennant) and his companion Donna Noble (Catherine Tate). These are River's only on-screen interactions with Ten, but she did meet him again in a handful of Big Finish Productions audio plays.

PROSE: The Trial of Doctor Who is the only valid story, as this Wiki defines it, that explicitly confirms that "∂³Σx²" is the Doctor's true name. However, this name has not been widely accepted.
Although there were parallels between Oswin and Clara Oswin, it was their similar deaths that sparked the Eleventh Doctor to begin his investigation into her identities - coining the "Impossible Girl" term after realizing that it shouldn't have been possible for him to run into the same woman when she had died twice.
Plot. Clara phones Danny, ready to admit the full truth of her travelling with the Twelfth Doctor and that she sincerely loves Danny. Danny is killed by a passing car midway through the call. Visibly upset, Clara schemes to use a hypnotic patch on the Doctor to blackmail him into saving Danny.
There have been several popular novels and short stories in the Doctor Who universe that explain these unaddressed issues, including one titled "The Turning of the Tide." No-one outside of the Doctor Who TV show universe (real life) knows the Doctor's true name. Even within the show's universe only a select few know it; River Song, other Time Lords, Clara Oswald at one point.
If any character in Doctor Who can be considered the Doctor's true love, it is surely Alex Kingston's River Song. Her history with the Doctor is complicated, since their travels through time meant they almost always met out of order.
It's established in the episode that River believed the Eleventh Doctor was the last given the regeneration limit. Since she thought it was an ironclad law, it's understandable that she wouldn't get the hint that this man she's never seen before is in fact the Doctor.
