Adrian's death improved the original story of Rocky Balboa by providing a richer and more heart-wrenching narrative centered around grief and purpose. Adrian's absence allowed for the exploration of Rocky's relationship with his son, creating one of the best aspects of the movie and a fitting end for the character.
Adrian died peacefully in her sleep on January 11, 2002, in Philadelphia, Pennsylvania, with her family along her side, aged 51. In the years since, Rocky, trying to break out of his bankruptcy, manages to open a small Italian restaurant, named after her.

Why didn t Adrian return in Rocky Balboa?
While promoting Rocky Balboa in a 2006 interview with Moviehole, Stallone explained that Adrian would've been much more one-dimensional in the movie's original storyline - which he didn't want.
The 76-year-old actor said in an interview that he did not participate in the filming of the movie because the story had been given a direction with which he did not agree. "That's a regretful situation because I know what it could have been," Stallone told THR about disagreeing with the new film's tone.

At the time of its release (2006), Stallone said he had a long conversation with Talia Shire, and she agreed and encouraged the new direction for the film without her character and Rocky's dealing with the grief of Adrian's death. His original first draft of the script featured Adrian in the film as usual.
Rocky tries to retire again, and the audience sees how Paulie becomes increasingly jealous that Rocky has money and fame now. Instead of asking Rocky for a job like one usually would, he tries to challenge Rocky to a fight and tear him apart verbally while drunk.
