One day, at a weapon demonstration in Afghanistan, the convoy Tony Stark was in was attacked by a terrorist cell called "The Ten Rings" and Tony was injured and kidnapped. In the attack, a cluster of shrapnel was lodged into Tony Stark's chest, nearly killing him, yet he survived thanks to his fellow captive Dr.
The fact that Iron Man snapped his fingers in the comics to manipulate millions of people while the MCU version did so to save trillions proves how insulting Iron Man's latest actions were to his MCU sacrifice.
Did Tony Stark ever get the shrapnel out of his heart?
Then in the third movie Tony learns that even if he doesn't have his suits, he is still Iron Man. At that point he decides that he does not require the Arc Reactor on him at all times to still be a hero, so he finally does the smart thing and has the metal removed from his chest.
While attempting to shut down this sentient armor, Tony Stark had a heart attack. The armor replaced Tony's heart with an artificial one to save his life, and Tony began steering himself away from such high technology out of fears of this incident happening again.
Stark had the arc reactor in his chest removed after receiving surgery to remove the shrapnel from his chest. He later removed the element and then threw the decommissioned unit into the ocean.
After regaining conciousness Tony Stark, using a box of scraps, built a small Arc Reactor, an almost limitless source of energy. He placed it inside his chest to power the electromagnet and no longer had to use the car battery.
When it was suggested early on in Tony Stark's time in the MCU that he get surgery to remove the shrapnel from his chest, he was decidedly against this. He found out that he would pretty much instantly die if such surgery were performed. Because of how the shrapnel had entered his body, it was impossible to remove.
In iron man 2008, pepper pots removed the magnet that was supposed to be keeping the shards from entering Tony's heart and then she replaced it with the upgraded version of the miniature arch reactor.
Would Tony Stark have been able to snap the gauntlet and survive if his suit had been made entirely of vibranium/uru? No. He had to be connected to the stones in order to use them and that connection would kill him if he used them all at once.
Hulk snapped half the universe back, and back to safety no less. Plus, he spent considerable effort wrestling with the Soul Stone to get Black Widow. By comparison, Iron Man just snapped away all of Thanos' army, a miniscule segment of the universe. Hulk survived his snap.
The important part of this insight, however, is that if Thanos always kept his word, then when he performed the Snap, he likely did indeed exempt Tony from it specifically since he had told Strange that he would spare Tony's life. That bit of honor proved to be his undoing.
During Tony's first conversation with Captain America since 2015's "Civil War," Tony gets very emotional and gets mad at Steve Rogers. He puts some blame on him for running away and not wanting to register as a superhero with the government to possibly prevent the Thanos snap in "Infinity War" from happening.
He gets a set of 'final words' as he, not Thanos, obtains the Infinity Stones: “I am Iron Man.” He gets his real final words in a very quiet moment with his wife – “Hey, Pep.” And he gets his last final words in the film in his pre-recorded message to the world, to his family, played during his funeral.
Pepper was telling Tony that her and Morgan, their daughter, would be okay since the Earth and the universe was safe from Thanos and that they had won the battle against him.
Stark's first arc reactor had two purposes: to keep the shrapnel from entering his heart, and to power his suits, which is still essential for his activities as Iron Man in the wake of Iron Man 3.

As Tony Stark/Iron Man, the actor looks to be taking his life-saving, power-generating arc reactor out of that hole and placing it back in, and it sure looks real and therefore kinda painful.
