It's not really a surprise she escaped with Nacho in that scenario. The epilogue shows they have a daughter together, while Massimo has given up and agreed to a divorce.
Does Laura end up with Massimo or Nacho in the movie?
It's revealed that Laura ended up with Nacho after escaping another abusive situation that ends up with Massimo drugging her and trying to get her pregnant. Laura eventually manages to escape him by seducing him and leaving when he lets his guard down.
Massimo and Adriano had always been rivals, and it was all part of Adriano's plan. Laura runs and she is shot by Anna and Adriano, who are in turn shot and killed by Massimo and Nacho. The movie ends with Nacho leaving and Laura lying in the arms of Massimo, her fate uncertain.
Massimo is still a mafia boss and still guilty of kidnapping, coercion, and a host of other criminal charges, but viewers can rest assured that he is no cheater.
In the book, Laura picks Nacho as Massimo tries to make her pregnant, so the fourth part might go in that direction. Anything could happen, and we are just waiting to find out. The creative team has not spilt the beans about the ending of the third movie, so we do not know who Laura chose.

As mentioned above, 365 Days 4 has yet to be confirmed by Netflix so there isn't an official release date. The second and third movies were filmed back-to-back which is why they were released fairly close to each other (April 2022 and August 2022).
He is just bad. All there is to say about Laura is that she's a victim. The idea that she 'fell in love' after having Massimo's junk repeatedly shoved in her face at every turn while he manhandles and manipulates her is likely triggering for some viewers.
Don Massimo Torricelli then gives Laura “365 days” to fall in love with him, before he frees her. As for how much Lipinska is in Laura, beyond the Sicilian vacation? Lipinska said much of the novels are based on real events in her and her friends' lives, but is reticent to identify which parts.
Critically panned, 365 Days still spawned sequels and satisfies viewers looking for similar steamy love scenes and complicated relationships. Movies like Fifty Shades of Gray and Basic Instinct offer similar problematic romances, while films like Y Tu Mamá También explore deeper themes alongside their steamy scenes.
