The film portrays Kevin's different personalities to be violent, destructive, and even cannibalistic, and in short, not an accurate representation of DID. Individuals with DID are not violent in most cases, rather they use their different personalities as a coping mechanism.
Is Fight Club an accurate depiction of schizophrenia?
While Palahniuk successfully puts us in the disorganized mind of the tortured narrator, he not only suggests a possibility of dissociative identity disorder, but also greatly exaggerates and generalizes schizophrenic symptoms and those of other psychological disorders.

The burn on the narrator's hand is like the movie's "cigarette burn," a symbol of a shift between two personalities. Tyler seems to be preparing the narrator for death. Tyler speaks glowingly about those who sacrificed themselves for the discovery of soap.
Why doesn't The Narrator have a name in Fight Club?
He does this as a nod to a series of articles he read that were written about the first person perspective of a body part. Edward Norton himself refers to the character as "Jack" because of this. In the screenplay, he is referred to as Jack.
Fincher pitched the role to Julia Louis-Dreyfus. The filmmakers considered Courtney Love and Winona Ryder as early candidates. Love claimed that she was cast as Marla Singer, but was fired after she rejected Pitt's pitch for a film about her late husband, Kurt Cobain.
Contrary to the theory, the real reason she and Tyler never appear in the same room at the same time from the Narrator's point of view is not because they are both alters but because when Tyler is active, the narrator is asleep.
