Later, after Katniss headed to the Capitol in order to end the war once and for all, it's revealed that Prim has also been allowed to go as a medic despite her young age on President Coin's orders.
By the events of the final movies, Katniss accepts her role as the symbolic Mockingjay, and she and her family realize what this might mean for them. Prim fulfills her dream of caring for others by being a nurse, and she goes in to treat the wounded when a series of bombs explode at the Capitol.
Prim hears Katniss and turns to see her running towards her. As Katniss gets nearer to the barricade, a second wave of bombs go off, sending Katniss hurtling through the air and killing many more children, including Prim. The last word from Prim in the novel before she died was "Katniss".

Keeping Katniss largely in the dark about the uprising's strategy also suggests that there are some unsavory elements to their tactics. It later comes to light that the design of the bombs that killed Prim likely belonged to Gale.
Reasoning: Yes, Gale designed a weapon, and this weapon went on and killed Prim. But this was war. Gale was trying to win, and he had no idea that Prim would be there.
