This is an extremely scary, graphic horror movie that should not be taken lightly. I have seen my fair share of horror movies over the years but this one definitely has a different feel to it. It is chilling, unnerving, and very difficult to watch sometimes.
It becomes clear that Rose's hallucinations aren't just in her head and that she is being taunted by a virus-like monster, or entity, that breaks its victims down before possessing them and using their bodies to kill them in front of a nearby witness.
It has some unsettling moments. I would give it a 6/10 for the scare factor. I would say the first part and the last part of the movie are the scariest.
Smile is one of the newest and scariest horror movies currently in theaters. The psychological thriller follows a therapist who, after witnessing the suicide of one of her patients, finds herself in the heat of disturbing and supernatural experiences.
In Smile, the demon moves through each witness of the smiling suicide and then to the next after 7 days. This chain effect adds to the fear factor by heightening the anticipation among audience members of when the next person will arrive, and who will die next.
This film is far from as graphic and disturbing as Ari Aster's “Hereditary” and “Midsommar,” but there are still some disturbing scenes that make members of the audience want to plug their ears and squeeze their eyes shut. The jump scares are also intense but there aren't so many as to overdo it.
The movie focuses on mental health and trauma and it does that very well. Not for the easily rattled, lots of gore and jump-scares. I would not recommend for kids.

One of the most heart-breaking casualties in Smile was Rose's cat, Mustache, but the film never revealed what actually happened to it. Smile follows Rose (Sosie Bacon), a doctor who gets cursed by an entity that presents itself through people with a sinister smile.
Smile's messy treatment of mental illness and trauma throughout one hour fifty-five minutes seems to be intentional and is bound to leave you feeling uncomfortable. The theme of inheriting mental illness and walking down a path that you have lived dreading all your life is poignant.
But the biggest sin of “Smile” is that it just isn't scary. The movie has to rely on fakeout jump scares like a burglar alarm and even opening a can of cat food because it knows it can't pull off scenes with actual danger or violence. A bad dream sequence can't be taken seriously because Rose would never act that way.
Blood and gore is infrequent but when it happens there is lots of it. Only 2-3 main scenes that show it. The film contains scenes in which human characters and supernatural creatures are engaged in violent struggles which include stabbings and being set alight.
Smile is rated R by the MPAA for strong violent content and grisly images, and language. Violence: People are seen cutting, stabbing, and burning themselves and others. A dead cat is shown.
