The horrifying presence attaches itself to Rose after she sees Laura (Caitlin Stasey), a patient with no history of mental illness, take her own life. Laura, who, just days earlier, was witness to a gruesome suicide, hurts herself while creepily grinning from ear to ear, making it all the more unsettling.
Rose Cotter (Sosie Bacon), a therapist who begins experiencing hallucinations after witnessing a patient take her own life. The patient, Laura (Caitlin Stacey), saw her professor die by suicide, after which she became the target of a presence with a sinister smile.
So once the psychiatrist started to suffer from similar hallucinations, Rose decided to investigate what was really going on. As it turns own, Rose is cursed by trauma, which in Smile is represented by a shapeshifting apparition that scares its victim until their mind is shattered.
Rose dies in Smile's ending because she was only able to overpower the entity once. She had enough fight in her to take on the gangly demonic monstrosity and even appeared to kill it, shattering a lantern over its head.
Smile clearly wants the audience to see the connection between Rose's curse and mental illness. Throughout the movie, characters use therapy buzzwords and often suggest that Rose's mental state is a result of her genetics and past trauma.
Rose initially panics, but starts smiling evilly, presumably because that her racist beliefs about black people would be right, as Chris would end up acting like a beast. Chris, either now unwilling or too exhausted to kill his former girlfriend, stops strangling her.
We learn that the young Rose allowed her mother to die rather than call for help because she was both afraid and angry with her mother. The demon believes this long-repressed revelation will paralyze Rose with guilt, making the task of subduing and overtaking her easier.
It is a demonic entity responsible for an ongoing series of gruesome murders, which it facilitates through a “curse” which passes from victim to victim in a “chain” stretching back to an unknown beginning.

One infected person is consumed, the entity forces them to take their own life and then the witness becomes cursed. But, what if multiple people witness that death?
What happened to Rose's father? The film's opening sequence pans across a series of portraits of Rose's family, with her mother, father, and her sister Holly all happy together. Then Rose's father disappears from the pictures. It's unclear whether he died or abandoned the family.

“You have wounds that never fully healed,” says Madeline, but Rose has already self-diagnosed: She is suffering, she insists, from “fleeting moments of stress-induced hallucinations.” This diagnosis comes along with a proposed solution. Rose demands a prescription for Risperdal, an oral antipsychotic medication.
With the colors, we wanted to do something unusual. We had read this study that was from back in the sixties where they found out that this sort of Pepto Bismol pink color somehow was supposed to make violent inmates less violent. So my production designer, Lester Cohen, and I, really grabbed onto that.
However, it goes deeper than being an 'evil creature', a curse, or a demon. The entity is something like Cthulhu. Hear me out; in the end of the film, when the entity's true form is revealed, we see Rose totally shut down. She stares, unable to comprehend, as rows of teeth and raw flesh is exposed.
If there were no one to see her die, no new trauma would be created, and the curse would be broken. While still not ideal, given that Rose would have to die, this is the only way to stop the monster for good.
