Laura begins screaming and panicking and Rose calls for help. Laura suddenly becomes unnaturally calm and smiles, then slits her own throat, terrifying Rose. The next day, another patient, Carl, smiles like Laura did and shouts to Rose that she is going to die.
The Smile movie explained the effects of trauma through a horror lens, featuring intricate and tragic themes that add real layers to the meaning of the movie. Following Laura's death, Rose starts experiencing visions of her own and begins seeing the smile-themed creature.
While suicide seems inevitable, there's another way to escape the chain and survive the creature's attack. And that is choosing from your own free will to scar another person's mind. While investigating the entity, Rose gets in touch with Robert (Rob Morgan), the only person who survived after being cursed by trauma.
The ending of the film is particularly sad since it proves that as soon as Joel agrees to help Rose figure out what's happening, he isn't able to extract himself from her, and he will never break free.

Even though Rose did everything she was supposed to do — that we're supposed to do to process our own trauma — the monster still won. The cycle will continue. Smile writer-director Parker Finn characterizes the two endings of his film as an attempt to get ahead of audiences' expectations.
[Editor's note: The following contains spoilers for Smile.] The smile curse operates in a fairly consistent fashion throughout the film. One infected person is consumed, the entity forces them to take their own life and then the witness becomes cursed. But, what if multiple people witness that death?
There's obviously a reason why the smile demon appears as a giant, looming depiction of Rose's Mom during the final act. One of the most unsettling moments in "Smile" takes place just before that when Rose's trusted therapist, Dr. Northcott (Robin Weigert) reveals herself to be the demon.
Joel arrives in time to see Rose pouring gasoline on herself and then lighting a match. The chilling final shot of Smile zooms into Joel's eye as he watches her burning to death. With Rose dying in a horrifying way, it continues the cycle and secures Joel as its next victim. Sorry, Joel.
Rose begins experiencing hallucinations induced by the demon. Shortly after, she has trouble finding her cat, Mustache, and its tragic fate is revealed during her nephew's birthday party. The boy opens Rose's present to discover her cat's dead body.
The entity (as Madeline) grins at her and says Rose's time is coming. Fearing she has no choice, Rose drives to her job with a knife. She goes into the building and finds Carl's room, where she goads him into freaking out. Morgan runs in and sees Rose stabbing Carl repeatedly.
She was smiling because she saw the police car and assumed Chris would get in trouble. I'm starting to think that she was using one of her mother's hypnosis tricks. That is, she'd "trained" Chris to love her when she smiled at him.
The book ends with Laura choosing to be with Nacho, but that is after Massimo holds her against her will, drugs her and tries to get her pregnant. It's not really a surprise she escaped with Nacho in that scenario. The epilogue shows they have a daughter together, while Massimo has given up and agreed to a divorce.
Laura returns home to her parents and confesses to them that she's in love with Nacho. When she adds that she doesn't know who she'd choose, her mother says women have to put their own happiness first in relationships. Laura dreams of a threeway with both Massimo and Nacho in which, yes, the men make out.

The demon can alter a person perceptions to mislead them into thinking they've managed to beat it, before resuming the haunting as before. As such, the only way to be rid of this entity is to pass on the curse which carries it on to another person.

The grief monster rips off its skin, revealing its horrible, true form, then snaps open Rose's mouth, crawling inside of her. Fully possessed by the demon, Rose douses herself in gasoline and sets herself on fire — just as the real Joel bursts inside her home and witnesses her death, becoming the next victim.
Smile clearly wants the audience to see the connection between Rose's curse and mental illness. Throughout the movie, characters use therapy buzzwords and often suggest that Rose's mental state is a result of her genetics and past trauma.
Ok so the movie Smile is based around a curse/entity that feeds and spreads through trauma. The main character Rose gets the Trauma both from her mother's death when she is young and also seeing her patient who had the Trauma die violently.
