At one point, her cat Mustache goes missing, only for the film to reveal that the cat had been killed, but in a delusional state, Rose seemingly wrapped up the dead cat to gift to her nephew at his birthday party. According to star Bacon, she doesn't believe that her character killed the cat.
The ending of the film is particularly sad since it proves that as soon as Joel agrees to help Rose figure out what's happening, he isn't able to extract himself from her, and he will never break free.
So, the entity uses Rose's body to pour gasoline all over the psychiatrist, lighting a match just as Joel comes through the door. Rose's body is smiling while she burns away, and we can see in Joel's eyes he will be scarred for life by that disturbing suicide.
As Joel walks in, Rose is wearing that familiar creepy smile and has covered herself in gasoline – about to light a match. So no, Rose does not survive in Smile and even worse, it seems the curse has found a way to keep itself going, with Joel now presumably set to be its next victim.

Ok so the movie Smile is based around a curse/entity that feeds and spreads through trauma. The main character Rose gets the Trauma both from her mother's death when she is young and also seeing her patient who had the Trauma die violently.

The demon can alter a person perceptions to mislead them into thinking they've managed to beat it, before resuming the haunting as before. As such, the only way to be rid of this entity is to pass on the curse which carries it on to another person.
According to the American Humane Association website, the female cat actor's name was Star, although it appears more than one cat was used during the filming (while eating the food, Mustache clearly has a notch in one ear, while the cat at the front door does not).

The monster in Smile passes from person to person like a virus, needing new victims in order to survive. As such, one possible course of action would be to remove oneself from society entirely, escaping to a place of isolation where there are no other human beings nearby to infect.

Most of the pictures are incredibly gory and bloody. May upset some. A woman accidentally bites her thumb creating a cut, some blood is shown. Blood and gore is infrequent but when it happens there is lots of it.
She realized she was back in her old home, confirming the entire brawl with the ghoul and her escaping was a hallucination, which pushed Rose over the edge. At this point, Rose embraced death and felt if she took her own life, the curse wouldn't last.
