When Padme confronted her husband on Mustafar, Anakin choked her to unconsciousness out of anger. While the act alone didn't kill her, Anakin's turn to the dark side was too much to bear. Holding on long enough to give birth to her children, Padme eventually passed away, having lost the will to live.
Because her final encounter with Anakin in Revenge of the Sith was so brutal and based on the medical droid's assessment that Padmé "lost the will to live," speculation has arisen that perhaps she simply died of a broken heart.
Amidala's body was taken to Naboo aboard Organa's ship, the Tantive III, and prepared by mortician Commodex Tahn, who made it look as if she had died pregnant. Her death was attributed to the alleged Jedi Rebellion that had prompted Order 66.

Did Anakin visit Padmé's grave?
At some point after his transformation into the armored Darth Vader, Amidala's husband, the fallen Jedi Knight Anakin Skywalker came to visit the mausoleum, stricken by grief and remorse for his part in her death.
Before Padmé's death, she still believed there was still good left in Anakin, and that one day he could be redeemed. During Anakin's time as the Emperor's enforcer, he continued to suffer from grief over Padmé's death, and guilt over his supposed hand in it.
Honestly, I just couldn't figure out who she was or why she was even in the funeral procession. This also means that for the longest time I forgot I could just look up the credits of the film. Long story short, she is Padmé's niece, Pooja Naberrie, and was portrayed by Hayley Mooy.
He did find out. Once he learnt of Luke's existence, Vader of course understood that Padme didn't die as a result of his Force choke. For the first time in decades, Vader had a motive to overthrow his Master.
The subtitles and script show some of Padme's final words to Obi Wan as “There is still good in him.” Every time I watch this I swear she says “Obi Wan... there's quite a name.” I have always thought of it as her recognizing him for everything he has done over the trilogy.

Anakin's Force Choke begins the series of injuries that ultimately leads to Padme's death, and convinces Obi-Wan to fight him. Despite this, when Anakin learns of Padme's death, his immediate reaction of grief and regret shatters everything in the lab with a powerful force shockwave.
Then, Vader also opened the Doors of the Dark Side and entered. Once inside the spiritual realm, Vader went through various trials, but eventually, he found Padmé. All indication was that Vader could have brought her back to life just like Momin did with his body. But Padmé refused to come.
In fact, both Vader himself and Padmé's last words were re-contextualized previously in Darth Vader #6 by Pak and Raffaele Ienco, where Vader discovers her dying words upon stumbling onto a holorecording of her death as seen in Revenge of the Sith.
The canonical comic Star Wars: Darth Vader #7 confirms the younglings' deaths caused Anakin grief, self-loathing, hatred, and pain, fueling his descent even further into the Dark Side.
She certainly knew something. The “I know. More than you realize, I know” line makes that clear, as does the Forces of Destiny short with Anakin, Padme, and Ashoka.
A major factor that contributed to Anakin Skywalker's fall to the dark side was the Jedi Council. The Council, led by Jedi Grand Master Yoda ,refused to give Anakin the rank of Master, despite being allowing him a seat among their ranks.
He had no idea Padmé had given birth before she died, still less that the Jedi had spirited the children away and hidden them. Darth Vader's illusions were shattered shortly after the destruction of the first Death Star.
By the events of Star Wars: Return of the Jedi, Luke had accepted that Vader is his father, and his hatred toward his father had completely disappeared, transformed into love and hope, like his late mother, Padmé Amidala, Luke believed there was still good left in Anakin and that he could be redeemed.

Anakin and Padmé met when Anakin was still just a young boy in Star Wars: Episode I – The Phantom Menace. Though he was already infatuated with her then, it was a boyish crush — one that Padmé could not reciprocate at the time.
