The original character was Dr Savik, a young male Vulcan scientist. So he'd have been referred to as "Doctor", not "Mister". In any case, he was only a character in a script treatment, not a full script. The only other reference to addressing female officers to date had been Spock once calling Uhura "Miss" in TOS.
In Star Trek II, Saavik is commonly referred to as "mister," a form of respectful military address. Normally applied to subordinates, the title is said in the film by Admiral Kirk and Captain Spock in reference to Saavik.

In a surprisingly tasteful scene for the early 1980s, Saavik and Spock get intimate. The result was going to be a child. However, since she was written out of the story in Star Trek VI: The Voyage Home, Saavik's pregnancy was dropped. One reason was that Nimoy objected to the idea.
T'Pring is arguably Spock's greatest Star Trek love interest, considering that she was once his fiancé. In Star Trek: TOS season 2, episode 1, "Amok Time", Spock (Leonard Nimoy) was forced to return to T'Pring (Arlene Martel) on Vulcan when he began undergoing the symptoms of Pon Farr, the Vulcan drive to mate.
