The Engineer was a cenobite who Joseph Thorne was hunting for while in Hell since he solved the Lament Configuration. The Engineer kills Daphne, Bernie, Thorne's parents, his partner Tony, and his wife and daughter. After killing each of the victims the Engineer leaves a child's finger behind.
Spencer explains his experiences in World War I caused him to see humanity and life as corrupt, leading him to use the Lament Configuration to summon the Cenobites, eventually joining their ranks. With Summerskill's help, Spencer's spirit escapes Limbo and re-merges with Pinhead.
His transformation into a Cenobite is similar to Dr. Channard's transformation from Hellbound: Hellraiser II. According to Serena Menaker in the beginning of the film, Roland Voight never does anything he can get someone else to do for him.

Is Pinhead in love with Kirsty?
All the evidence to me shows that Kirsty and Pinhead admire and do to an extent love each other. A kind of Romeo and Juliet thing going on, but even more of a horrific way of being in love.
Roland seemingly gets pulled into the Cenobite realm after he chooses the Leviathan configuration. In Hellraiser's closing scene, a powerful invisible force strips his skin and puts needles in his throat, which indicates that he, too, has been turned into a Cenobite.

The original book that it's based on, The Hellbound Heart, was suggestive of a female Pinhead, and the comic books also explored the idea. There's also plenty of fan art, wonderful works that all theorized the possibility of Pinhead as a woman. So it only felt appropriate to us.

Because of the iron pins that has been nailed deep into his head, Pinhead is assumed to be in a perpetual state of pain, a pain that he has since his transformation from human to cenobite gotten used to and seems to derive pleasure from it. This allows him to endure extreme amounts of pain without slowing down.
Leviathan is obsessed with law and order, and views the human realm of flesh as chaotic and wrong. It uses the Cenobites as its foot soldiers in the war against chaos and flesh. The Labyrinth which it presides over represents the maze of the mind.
What is the Lament Configuration Box? The Lament Configuration Box is a cubic puzzle that people need to solve to summon the Cenobites. Since the Cenobites come from a dimension of unbounded pleasures and pain, people solved the Lament Configuration Box hoping to experience things unknown on Earth.
In the best-case scenario, it would have brought Matt back in the flesh and in the flesh only, leaving Riley with an uncanny husk of the brother she knew and loved. More likely, it would have doomed him to an eternity of pain.
In the final scene, Roland Voight (Goran Višnjić), an incredibly wealthy character, becomes a Cenobite. This ending works because there could be another Hellraiser movie in the future and perhaps Voight would become a villain.
While taking Nora to the hospital in Trevor's van later to treat her injury, Nora is abruptly pulled into The Labyrinth where Pinhead mutilates her with her hook-chains and taunts her when she prays to god only to flay her and turn her into a cenobite.
+++SPOILER+++The Six Configurations+++SPOILER+++ The configurations that get fleshed out in the movie are Lament/Life, Liminal/Sensation and Leviathan/Power. I am pretty sure that Lazarus/Resurrection is a callback to the first two movies. Matt would've returned as a skinless zombie, like Frank or Julia.
