Darth Malak, voiced by Rafael Ferrer, is the old Sith apprentice of Revan and the main antagonist of the first game. As Jedi, Malak and Revan disobeyed the Jedi Council and helped the Republic defend against the Mandalorians.
Taking Malak as his Sith apprentice and using the Star Forge to construct a military fleet and weapons, Revan waged war against the Republic in the resultant Jedi Civil War. However, Revan was betrayed by Malak and was captured by the Jedi Knight Bastila Shan, who saved Revan and formed a Force bond with her captive.
In Legends, Revan was a Jedi Knight who broke away from the Jedi Order, became a Dark Lord of the Sith, and formed a Sith Empire that waged war against the Order and the Galactic Republic. While experiencing amnesia, the former Sith was retrained as a Jedi to confront the Sith Empire, Revan's own creation.

Training in the ways of the Force under the mentorship of Kanan Jarrus, Ezra became a Jedi Padawan and a part of the emerging rebellion. He also grew close to Hera Syndulla, Zeb, Chopper, and Sabine Wren, who was like a sister in his new found family.
Disney used Revan as an inspiration to create Kylo. His armor, inner conflict between Light and Dark and his use of the Force. Snoke considers Kylo to be the ideal embodiment of the Light and Dark sides of the Force, much like Revan, who was a master at both sides of the Force.

Ten years after her death during the Mandalorians' genocide of the Cathar people on their homeworld in 3973 BBY, the mask was claimed by the Jedi Knight Revan and was immediately donned by the Jedi.
Sometime after Rebels, he returned to Lothal and soon discovered the ancient tomb of Darth Revan. It was from there he soon discovered he was the descendent of the legendary dark lord that he took on his mask, armor and light saber claiming the mantle himself.

Unfortunately for Darth Malak, Revan did not take kindly to being called soft by his subordinate and resolved to prove Malak wrong by cleaving his jaw off with a lightsaber, bequeathing his Sith underling with his iconic mutilated face that would become a central part of the characters design.

The mask belonged to a Mandalorian woman on Cathar during the Mandalorian Wars. She took part in the Battle of Cathar under Cassus Fett. The woman knew that Fett was going to commit genocide on the Cathar people and tried to convince Fett to spare them.
History. Revan constructed this lightsaber during his retraining, and made it with a blue blade. After the defeat of Darth Malak, Revan changed his blade's color to green, using this until his capture by the Sith Emperor.
Some time before 3,963 BBY, Revan, along with Alek, attained the rank of Jedi Knight. The two friends were acknowledged as being among the Order's most promising members, although Revan was always recognized as the leader—the more powerful and intelligent of the pair.
As an in universe explanation I'd say coincidence. We know he was obsessed with Vader, so my guess is he tried to make a helmet based off of Vader's and it happened to look like Revan's. It's possible he was inspired by Revan too but I think it is unlikely simply because of how much he wanted to be like Vader.
In Tales of the Jedi, set thousands of years before the original Star Wars film, the Mandalorians are a major military power who side with the Sith in their war against the Jedi, and their leader is manipulated by the Sith into triggering a war with the Republic.
