In "Is That You?", visiting a much more elaborate version of Prismo's Time Room, created as a "Plan B," Finn makes a choice to save Prismo by creating a paradox that vaporizes his dream world duplicate. Instead of turning to dust, Finn's body turns into a sword, dubbed the Finn Sword by Prismo.
When Finn uses the 4-D golden sword to attack the black hole he created, Finn's golden sword is sent into the Cotton Candy Forest by the explosion. Finn is without a sword until "Mystery Train", which could mean he lost it after the explosion. In the video game Adventure Time: Hey Ice King!
When the Citadel Guardian's blood got on the stub where Finn's arm once was, his arm didn't grow back, but it instead left behind a flower, which was likely a remnant of the sword. In "Breezy" it's revealed that the condition of the flower is based on Finn's emotional state.

Who was Finn reincarnated as?
Shermy is confirmed to be a reincarnation of Finn, which makes sense considering their similarities and the fact that Finn can resurrect when he wants and be another living being due to being an incarnation of the Blue Catalyst Comet.
The Demon sword first appears in the episode "Dad's Dungeon." In the episode, Finn and Jake go to Joshua's Dungeon to retrieve Jake's family sword. At the end of the dungeon Finn finds it encased in locks and stone. He breaks it loose and uses it to fight the final monster of the dungeon.
At her tree, Jake teases Finn again about liking the Wizard, but Finn denies it and goes to sleep. In the morning, Huntress Wizard takes the two to a magic spring to drink from so they can be immune to the magic boar's lightning powers.
Finn was, for a long time, the only confirmed human in the Land of Ooo, to the point where he had an epithet, "The Human", and was referred to by many as "Finn the Human". The reason for this is revealed in the episode "Her Parents," as Lady Rainicorn's dad believed that humans were extinct.
During Adventure Time, it was revealed that Finn and Jake were the same age, and the series ended when Finn was 17. Therefore, it would only make sense that Jake would pass away by the time Finn reached adulthood.

In Adventure Time Mini-Series: "Islands" it revealed that around thousand years prior to the main events of the series, a group of human survivors of the Mushroom War fled Ooo on a container ship (as seen in Stakes) to escape from the vampires, oozers, and other magical and mutated dangers.

Finn dances with Quilton's daughter, Roselinen. However, no one knows how to get him home. Years later, Finn and Roselinen have married and had two children, Bonnie and Jay. Quilton tells them that an ancient book has been discovered revealing that the door to Finn's world appears periodically.
In "The New Frontier," Jake says that he will probably live another hundred years suggesting that his magical powers also make it so Jake will live much longer than the average dog.
However, Finn seems to hate romance movies and kissing because in "Go With Me," he actually vomits during a kissing scene in a movie. However, at the end of "Dream of Love," Finn was not grossed out by Tree Trunks and the Pig kissing.
Finn, still on the raft, survived and washed up on the shores of Ooo. Finn ended up in the woods and was found by Jake's parents, Joshua and Margaret, who raised him as their own alongside Jake and Jermaine.
With the Lich disposed of, Finn pursues his father, who is hitching a ride with escaping criminals on a loose piece of the Citadel, still held on by a vein. One of the criminals cuts the vein, but Finn holds on, causing his grass sword to envelop his arm and eventually rip away, severing it.
He stopped using it after receiving the Demon Blood Sword and did not use it again until "Normal Man," after the Finn Sword was destroyed. He keeps it in a makeshift pocket on the side of his backpack. It can be seen again in Issue 1 (although the comics are of dubious canonicity).
The Grass Sword was Finn's sword from the episode "Blade of Grass" which replaced Joshua's broken demon blood sword. He buys it from the Grassy Wizard, who cursed the sword so it will bond itself to its owner for eternity.
