Dumbledore told Harry in Half-Blood Prince that Tom Riddle's diary had been his first clue that Voldemort had created a Horcrux and that there was likely more than one.
Most of the people thought the chamber of secrets was a myth. Voldemort had given his diary to Lucius and asked him to take care of it. Lucius thought it was something important but didn't know it was an horcrux or that it would unleash a monster.. He merely did it because Arthur Weasley was conducting raids.
In fact, the diary had the ability to take hold of Ginny's strength, even nearly killing her. The diary was the first of Lord Voldemort's Horcruxes to be destroyed, by Harry Potter in the Chamber of Secrets, who stabbed it with the fang of the very Basilisk that was used to commit murder in the first place.
He poured the piece of his soul that resided in the Horcrux diary inside of Ginny and fed secrets to her that only that piece of Voldemort's soul would know. The more Ginny used the diary the stronger Tom became, using her fears and secrets as nourishment, if you will.

Due to his humiliation in the Triwizard Tournament, Cedric eventually becomes a Death Eater and kills Neville Longbottom. Due to Neville's death, the final Horcrux Nagini was never destroyed, and as a result Voldemort is never defeated, kills Harry and takes over the Wizarding World.
In the end, he renounces Voldemort's cause by putting Draco's safety first, running away from Hogwarts with his family. Because he betrays Voldemort, his crimes are forgiven, and, again, he is not sent to Azkaban.

He knew. Lucius was arrested at the end of the first war with Voldemort but convinced the authorities that he'd been Imperiused. It's possible that any evidence that would have proven otherwise was provided by Snape, whose cover Dumbledore couldn't risk.
