Being the Black's house elf, when Sirius died and gave all his belongings to Harry, Kreacher became Harry's, too. A house elf must do what his/her master tells him/her to do, otherwise (s)he will have to punish him/herself. Kreacher didn't have any problem obeying Harry, even though he still called Hermione 'Mudblood.

Narcissa and Bellatrix were kind to Kreacher because he was very much loyal to them, he followed each and every order of them. Also, Kreacher was not a muggle lover or blood traitor. So, there was no reason ro be harsh towards him.
