Was Tony scared of Thanos?
Since Iron Man is so smart he's bound to have some pretty big rivalries and enemies across the universe, and perhaps his most feared rival is the Mad Titan himself, Thanos, who happened to be the first villain to actually defeat him and complete his mission.

Thanos is known for being superlatively powerful, but he's also as intelligent as they come. Thanos was educated on Titan among his fellow Eternals and learned many secrets of the universe in his travels. Iron Man is intelligent, but Thanos is smarter than Iron Man could ever be.
Of course, Thanos had underestimated his enemy. Tony Stark had one last tool in his armor; sophisticated medical technology. Stark will have been badly injured, likely suffering severe internal bleeding, but he survived, the wounds closed with some sort of sealant.

What does Thanos dislike? Thanos hates Deadpool. While Thanos only has eyes for Mistress Death, she, on the other hand, may have eyes for another. Everyone's favorite Merc with a Mouth has come in contact with Mistress Death a number of times (basically every time he “almost” dies, which is quite a bit).
Stark had had visions of death and destruction in Avengers: Age of Ultron and presumably before and after too, but the threat he sensed coming to the earth did not materialise in his mind in the form of Thanos, though he did know it came from the outer space. Thanos, though, knew who exactly Stark was.
“People have speculated whether there was some fear on the Hulk's part about having to face Thanos again,” Russo said. “But I think ultimately what it is, is that he's tired of playing hero to Bruce Banner.”
Moreover, he was scared of the effect it would have on his own psyche since Thor's inability to stop Thanos meant he had to also confront the fact that he had failed to save many other people.
Hulk goes up against Thanos in a fight but is defeated by the Mad Titan. Though it ultimately wasn't surprising that Thanos would beat Hulk, it was shocking to see the easy fashion that he did it in. It could even be assumed that Thanos used the Power Stone that he had.
Assuming Tony had survived Avengers: Endgame, the confrontation with Thanos would have still left him with medical problems. If he survived using the Gauntlet, he may have even lost his hand. He certainly would have needed long-term medical attention.
In Iron Man #6, written by Christopher Cantwell with art by Cafu and Frank D'Armata, Tony has to be injected with opioids to save his life. As a result, he becomes addicted to morphine. Iron Man #19 by the same creative team reveals that Tony kept using the drugs "to keep going, stay strong, not los.
The arc reactor keeps Tony Stark alive. In the different versions of Iron Man, Stark's group has been ambushed in Vietnam, the Persian Gulf, and Afghanistan. Stark sustained serious injuries as a result. He created the personal arc reactor to generate energy for an electromagnet keeping shrapnel away from his heart.
Tony Stark has a damaged heart from the plane crash that the Mandarin caused, and because of that, he has an implant made by Dr. Ho Yinsen that keeps it beating.
