I was surprised to learn that the actress playing Rachel in those scenes in 2049 is not current day Sean Young that's de-aged, but a younger actress who looks very similar to a young Sean Young, with the mo-cap sensors on her face.
For Blade Runner 2049, Nelson and company sought to recreate actress Sean Young as faithfully as possible to her 1982 appearance as the replicant Rachel. The team went to great lengths to achieve this near-perfect double.
Because of this memory, K became convinced that he was Rachael and Deckard's child, but was eventually informed by Freysa that he was not and that the child was female. With this revelation, K soon determined that Stelline was the child.

How did Deckard get Rachel pregnant?
The film reveals that Deckard was able to conceive a child with Rachael, and this was possible because she was an experimental prototype (designated Nexus-7), the first and only attempt to design a replicant model capable of procreation.
Rachael wasn't a human as her eyes glowed as only a replicant's eyes do and she failed the Voight-Kampff test when Deckard tested her, although it took more questions over a longer period of time for the test to determine that Rachael was, in fact, not a human.
The big twist in the final act of the movie is that K isn't the son of Deckard and Rachael at all, but just another cog in the machine that might lead to the child's discovery.
However, he later learns that, even if the memory is real, it was not his own but Dr Ana Stelline's. Through her work, Dr Stelline shared her memory with K, and this allowed him to understand what it was like to walk in her shoes, but most importantly, to believe himself a free, loved replicant.

What happens to Rachel at the end of Blade Runner?
The theatrical version ended up giving audiences a rather typical Hollywood-style ending. Deckard runs away with Rachael after Roy's death. There's even some off-camera narration in which the blade runner explains that Rachael outlives her predetermined lifespan.
Gaff could have caught Deckard and Rachael, but his sympathy for their cause influenced him to allow them to escape, and that's exactly the type of person Cal was looking for.

How did they bring back Rachel in Blade Runner 2049?
Actress Loren Peta was brought on as the Rachael body double. With Nelson and Young also on hand, the scene was done with dots all over Peta's face, which would be needed when the footage went through the motion-capture phase. Peta's face would be erased, and CGI Sean Young would be inserted.
Stelline informed K that it was illegal for real human memories to be implanted into replicants. She offered to analyze his memory and tearfully confirmed that it was real.
The horse, with the inscription "6.10. 21", establishes the connection between the dead female replicant who gave birth and died on 6.10. 21, and K himself, implying that the memories were not in fact "implanted" but are rather genuine after all.
Wallace himself wanted to discover the secret to reproduction in replicants in order to support interstellar colonization, and to this end he sent Luv, now his assistant, to steal Rachael's remains from the LAPD headquarters and follow K to find Rachael's child.
K started the movie believing he was made, with implanted memories. He then thought he was born, and that the memories were real. In fact, as revealed by the end of the movie, he was made and the memories were implants – but they were real, implanted memories from the actual daughter of Deckard and Rachael.
What happened to Joe at the end of Blade Runner 2049?
On his way, he encounters a large holographic advertisement for Joi, his holographic companion. While capturing Deckard, Luv (Sylvia Hoeks) destroys the emanator of K's Joi, effectively killing her in the process.
