The ending of the Arkham City game saw the shocking death of the Joker, ending with Batman tenderly carrying Joker's body out of the site of their final battle.
In "Arkham Knight," the Joker is actually dead. The evil clown you encounter is in fact a hallucination, a consequence carrying over from the conclusion of "Arkham City," where Joker infected Batman with a toxic sample of his own blood.
Earlier in the game, Joker fakes his own death in a way that completely fools Batman's hi-tech cowl, showing an accurate skeleton and making the cowl display Joker's status as "deceased". Then he pounces and gasses batman from behind, and we don't see that fake Joker again.
Joker is put in the electric chair and acts like he's got stuff to do so wrap it up, B! Minutes after his death, his goons sneak in, find his body, and inject it with some special kind of serum that awakens the very recently deceased. Joker pops back up and continues his life of crime.
"Endgame" restores the Joker's face, and also reveals that he knows Batman's secret identity. The story ends with the apparent deaths of Batman and the Joker at each other's hands, though it is revealed that they were both resurrected in a life-restoring Lazarus Pit, without their memories.
