Essentially, you discover at the end that the doll Brahms was never 'alive' to begin with – nothing supernatural was occurring. Instead, it turns out that a 30-year-old man has been living in the walls moving the doll around to make it seem like it's haunted.
Jude, the boy who moves into the Heelshire house, becomes obsessed with the Brahms doll, which is revealed to hold a demonic entity inside it (despite having been smashed completely and showing no demon whatsoever in the first film) when the doll's head is destroyed.

Why did they hide Brahms in The Boy?
After which, he started a fire so as to fake his death and avoid punishment, which resulted in his face becoming horribly burned and scarred. To protect their son, the Heelshires hid Brahms away in the walls of their mansion and had him declared dead, so that no one could find him.

The final act of the film involves the Brahms doll being destroyed and a much more demonic and rotten version of Brahms lying beneath the pristine porcelain demeanor. This releases the evil spirit, but the family still manages to defeat it.
Somehow “Brahms: The Boy II” manages to at least look better than its predecessor. Everything else works as needlessly as the lengthy title. It is indeed a sequel of sorts to 2016's bizarre and wacky “The Boy,” where a porcelain doll in a suit tormented its babysitter.

The entire film seems haunted by the original. Viewers who enjoyed the original one enough to watch the sequel will probably be disappointed. And viewing the sequel on its own won't do the film many favors, either.
Greta doesn't want to leave him, but he runs at Brahms to hold him off. Brahms is huge and overpowers Malcolm. He smashes his fist in Malcolm's head, and Malcolm is knocked out. Brahms heads towards Greta, telling her in a small child voice for her to stay.
