[Editor's note: The following contains spoilers for Smile.] The smile curse operates in a fairly consistent fashion throughout the film. One infected person is consumed, the entity forces them to take their own life and then the witness becomes cursed.
Ok so the movie Smile is based around a curse/entity that feeds and spreads through trauma. The main character Rose gets the Trauma both from her mother's death when she is young and also seeing her patient who had the Trauma die violently.
The ending of the film is particularly sad since it proves that as soon as Joel agrees to help Rose figure out what's happening, he isn't able to extract himself from her, and he will never break free.
After investigating the string of previous suicides, Rose learns that she's doomed to kill herself in front of someone else, which will pass along the curse to that person. Or, she can kill an innocent person and be freed from the ghastly grief-monster's hold on her.
Not only is Rose unable to defeat the demon in Smile's ending but, in a truly vicious twist, the entity waits until Joel arrives before possessing and killing her. This means that it will be able to live on in him since the curse needs a witness to pass itself on.
It is a demonic entity responsible for an ongoing series of gruesome murders, which it facilitates through a “curse” which passes from victim to victim in a “chain” stretching back to an unknown beginning.
It is a sadistic and violent demonic entity that feeds off of the trauma of its victims, tormenting them to sustain itself, but also because it enjoys causing its victims to suffer, until they eventually kill themselves, or until it possesses them and forces them to kill themselves.
There, the entity takes the form of her mother, revealing that, after overdosing, she asked a then 10-year-old Rose to call for help. Rose declined and her mother died. Rose forgives herself at that moment, but that doesn't deter the creature, which transforms into a monstrous version of Rose's mother.
She pours it on herself and lights a match. After spending our time seeing things from Rose's perspective, often in uncomfortably closeup shots, we see her final moments through Joel's eyes, who can't look away despite his horror. The curse has now been passed on to him.
Then, in the very last sequence, the demon removes its face to reveal its true appearance — a pale, lifeless entity with a seemingly endless grin. When the creature literally crawls inside Rose and completely takes her over, we're led to believe that this may have actually happened.
We learn that the young Rose allowed her mother to die rather than call for help because she was both afraid and angry with her mother. The demon believes this long-repressed revelation will paralyze Rose with guilt, making the task of subduing and overtaking her easier.
What happened to Rose's father? The film's opening sequence pans across a series of portraits of Rose's family, with her mother, father, and her sister Holly all happy together. Then Rose's father disappears from the pictures. It's unclear whether he died or abandoned the family.
With the colors, we wanted to do something unusual. We had read this study that was from back in the sixties where they found out that this sort of Pepto Bismol pink color somehow was supposed to make violent inmates less violent. So my production designer, Lester Cohen, and I, really grabbed onto that.
Smile clearly wants the audience to see the connection between Rose's curse and mental illness. Throughout the movie, characters use therapy buzzwords and often suggest that Rose's mental state is a result of her genetics and past trauma.
Rose begins experiencing hallucinations induced by the demon. Shortly after, she has trouble finding her cat, Mustache, and its tragic fate is revealed during her nephew's birthday party. The boy opens Rose's present to discover her cat's dead body.
Smile 2 has been confirmed by Paramount following the runaway success of the first Smile film, which was released back in September 2022. A sequel to Smile has been confirmed following the incredible success of the first movie.
So no, Rose does not survive in Smile and even worse, it seems the curse has found a way to keep itself going, with Joel now presumably set to be its next victim. Smile is currently playing in UK cinemas.

After causing the most inner turmoil that it possibly can, the unnamed monster kills its victim and leaps into the most traumatized witness to continue the trend. However, the film implies that dealing with one's trauma healthily could be the most effective way of killing the monster once and for all.
The only way to get rid of it and survive its hunt is to kill someone else, while a 3rd party is watching it happen, causing it to lose interest in its original prey and go after said third party, meaning that it still wins either way since the victim is eventually driven to suicide by The Smile Entity, or they are ...
The Evil Grin is a very common theme in fiction dealing with villains, and is comparable to the Evil Laugh. Some antagonists do this when their plans are working, they reveal their true nature, or most importantly, while manipulating someone else.
