During the feast, Thresh saves Katniss when she is pinned by Clove, killing the assailant before asking Katniss about Rue. Thresh is touched that Katniss had taken care of the young girl, and he feels that he owes her. As a result, he spares Katniss's life and warns her to run from Cato.
Rue ultimately saves Katniss in this chapter. Katniss' friendship with Rue keeps Thresh from killing her. He feels as if he owes Katniss for her kindness toward Rue and, indirectly, his district, and Katniss of all people understands not wanting to owe anyone.
During the feast, Thresh saves Katniss from being killed by Clove. After Thresh avenges Rue's death by killing Clove, he asks Katniss about Rue. Thresh is touched that Katniss had taken care of Rue and tried to protect her from the other tributes, and feels that he owes her.
But when Peeta found Katniss weakened and disoriented by the wasp venom, he saved her rather than choosing to kill her or lead the Careers to her. Peeta clearly still feels a sense of loyalty to Katniss, and he likely has genuine romantic feelings for her.
Had she lived, Thresh likely believed, they could have become allies and both arrived home at District 11, victors and wealthy—which meant their families would never know hunger again. It's very likely. He spared her life because she killed the boy who killed Rue and helped Rue die in peace.

An announcer informs the tributes that something they each need at a 'feast' (for D12 – Peeta's remedy). Katniss risks her life to get it for Peeta. She is almost killed by a girl from D3, but is rescued by Thresh – Rue's fellow District 11 tribute. 'Just this one time, I let you go.
Clove pins her, taunting her all the while, and says they're going to kill her like they did her ally, Rue. But just as Clove cuts Katniss's lip, Thresh, the boy from District 11, grabs her.
A boy named Thresh was chosen as the male tribute from District 11 when Rue was selected for the female slot, and Katniss doesn't see him much during the Games until she is attacked by Clove, another tribute who teamed up with Cato and others.
Katniss feels betrayed, most of all by Haymitch, who used her and Peeta. Eventually she wakes to find Gale there. He's badly injured and tells her the Capitol bombed District 12 after the Games. He got Prim and Katniss's mother out in time, but the District 12 is gone.
This suggests that Katniss loves Peeta, and not just because she feels that she owes him her life. Instead, Katniss wants to protect him and bring him home because she has strong feelings for him, even though she isn't sure what those feelings mean.
When Clove saw Thresh holding a large rock, she screamed for Cato. Thresh smashed the rock into her temple, denting her skull. Cato ran to her after hearing her cries, but Clove died in Cato's arms as he begged her to stay with him.
It is unknown why only Clove went in for the backpack, but by the time she is fatally wounded by Thresh, Cato is too far away to save her. As Katniss flees the feast, she sees Cato kneel beside Clove and beg her to stay with him.
