It had been demonstrated that even with wands, wizards had been unable to break spells by house-elves. Dobby using his magic to Side-Along Apparate House-elves could Apparate even in places where wizards and witches could not.
It was the guilt he felt that made him hurt himself. I take it as a sign of how the Malfoys treat him. Anytime Dobby does something wrong, the Malfoys make him punish himself, such as when they made him iron his hands for being absent, or flog him visciously for letting their dinner burn.
